This program is designed to assist state, local, and tribal governments enter into outcomes-based contracts to reduce recidivism and homelessness (as measured through the number of nights housed in permanent supportive housing and reduction in number of nights in emergency facilities). Contracted services include “reentry services which are tailored to individuals leaving incarceration who are at moderate to high risk for recidivism in the community and services available to individuals living in permanent supportive housing units.” (p. 5)
